I made the attached flyer recently when I taught this at a class. I "modified" the instructions from the link Julie posted, is all. I hope this helps! :-)

I just posted this to the Techniques & Templates forum; some have been able to access it and some have not... still waiting on an answer as to why that is, but maybe this will help some of you out!

Update!
It seems the issue with opening the file is whether or not you have the latest version (7.0) of Adobe reader. See the link for details and have fun making these neat cards!
