Here's the template. Solid lines are cut out, dashed lines are scored. Circles are punched out. Basically just stamp it, fold it, flip the top flaps over so it resembles a 1-d flower pot, and tie them shut with some string. In order to make the candy stay inside, I just taped it. I managed to fit 3 'fun-sized' treats inside without it bulging.
The attachment is just a .jpg. It's not at actual size, but all the right measurements are on there. You should be able to fit about 2 of these on a sheet of paper.
